2/7/2023 EnchantaVaughn Approved Upon arrival to the school setting, the client appeared to be in a content mood as evidenced by greeting QMHP-C with a moderate. During morning observations, QMHP-C observed the client displaying on task behaviors as evidenced by following directives provided by his teacher and positively engaging with peers. During midday observations, QMHP-C observed the client engaging with peers during instructional period. During afternoon observations, QMHP-C observed the client regaining focus as evidenced by following school rules and engaging with peers appropriately during instructional period. QMHP-C greeted the client upon his arrival to the school setting. QMHP-C encouraged the client to continue to make good decisions throughout the day. During Midday observations, QMHP-C redirected the client to pay attention during instruction periods. QMHP-C engaged the client in discussion surrounding the importance of paying attention and following directives provided by his teacher. QMHP-C praised the client for completing his day achieving his goals. Upon arrival to the school setting, the client communicated that his morning was going well. The client responded well towards the interventions as evidenced by positive feedback and eye contact. During morning observations, the client explained that he would achieve all objectives throughout the day. During midday observations, the client demonstrated the ability to regained focus. During afternoon observations, the client was able to complete all ISP objectives as the remainder of the school year continues in an effort for him to reach all of his goals to successfully discharge from services.
